By the time you're done, you'll have your plan, understand how to make changes when needed, and be able share it with the right people, including family members or non-clinical caregivers.

You can meet with an ACP specialist as many times as you want.

What does this cost?

Most insurance plans, including Medicare, cover Advanced Care Planning (ACP) services. Depending on your specific coverage, a copay or deductible may apply. There are NO additional charges from us.

What if I already have an advanced directive?

That's great! An advance directive (AD) can be one part of the plan. But it's not the full picture.

With an ACP you document WHY and WHEN you would want certain care to take place.

It gives your care team more information to make the decisions you would want.

What if I change my mind?

You always have the option to return to your plan and update it to best reflect you.

Healthcare professionals typically recommended you review your plan once a year and discuss any changes with your care team.
